NOTICE OF PRIVACY PRACTICES
THIS NOTICE DESCRIBES HOW HEALTH INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
I. My Pledge Regarding Health Information
I understand that health information about you and your health care is personal. I am committed to protecting health information about you. I create a record of the care and services you receive from me. I need this record to provide you with quality care and to comply with certain legal requirements. This notice applies to all of the records of your care generated by this mental health care practice. It will tell you about the ways in which I may use and disclose health information about you. I will also describe your rights to the health information I keep about you and outline certain obligations I have regarding its use and disclosure.
I am required by law to:
• Make sure that protected health information (“PHI”) that identifies you is kept private.
• Give you this notice of my legal duties and privacy practices with respect to health information.
• Follow the terms of the notice that is currently in effect.
II. How I May Use and Disclose Health Information About You
Federal privacy rules allow providers to use or disclose PHI without written authorization for treatment, payment, or health care operations. This may include consultations, referrals, or coordination of care. Disclosures for treatment are not subject to the minimum necessary standard.
Lawsuits and Disputes: If you are involved in a lawsuit, I may disclose PHI in response to a court or administrative order, or other lawful process after efforts to notify you.
III. Certain Uses and Disclosures Require Your Authorization
Psychotherapy notes, marketing purposes, and the sale of PHI require your explicit authorization except under specific exceptions outlined in HIPAA and Nevada law (NAC 641A.243).
IV. Certain Uses and Disclosures Do Not Require Your Authorization
I may use or disclose PHI without authorization for reasons required by law, reporting imminent threats, public health activities, oversight, judicial proceedings, law enforcement, coroners, research, specialized government functions, workers’ compensation, appointment reminders, treatment alternatives, or health-related services.
V. Certain Uses and Disclosures Require You to Have the Opportunity to Object
I may share PHI with family, friends, or others involved in your care unless you object. Consent may be obtained retroactively in emergencies.
VI. Your Rights Regarding PHI
You have the right to:
• Request limits on PHI use/disclosure.
• Restrict disclosures if paid in full out-of-pocket.
• Request specific contact methods.
• See and get copies of PHI (except psychotherapy notes).
• Request an accounting of disclosures.
• Correct or update PHI.
• Obtain a paper or electronic copy of this notice.
Acknowledgement of Receipt of Privacy Notice
Under HIPAA, you have certain rights regarding the use and disclosure of your PHI. By checking the box below, you acknowledge that you have received a copy of this HIPAA Notice of Privacy Practices.
BY SIGNING BELOW I AM AGREEING THAT I HAVE READ, UNDERSTOOD, AND AGREE TO THE ITEMS CONTAINED IN THIS DOCUMENT.
Consent for Telehealth Consultation
In accordance with Nevada statute (NRS 629.515):
1. I understand that my provider wishes me to engage in a telehealth consultation.
2. My provider has explained how video technology differs from in-person visits.
3. Benefits include easier access and convenience.
4. Risks include interruptions, unauthorized access, or technical difficulties.
5. If connection fails, I agree to be contacted by phone.
6. I must be located in a state where my provider is licensed and state my location each session.
Consent to Use Telehealth by SimplePractice
Telehealth by SimplePractice is the platform we will use. By signing, I acknowledge:
• It is not an emergency service; in an emergency call 911.
• The service does not provide medical advice.
• My provider may not have technical access to all information in the platform.
• I will not share my session link with unauthorized individuals.
Informed Consent for Psychotherapy
You will be treated by a Registered Intern working under the supervision of a Qualified Supervisor in Nevada (NRS 641A.310). By participating, you consent to the sharing of information with supervisors for training and quality of care purposes. All clinicians follow HIPAA and Nevada ethics codes.
Confidentiality
All session content is confidential except when required by law, including:
• Threats of suicide or harm.
• Threats of harm to others.
• Suspected abuse of children, elders, or dependent adults.
• Court orders or subpoenas.
• Treatment mandated by court order.
If we see each other in public, I will not greet you first to protect confidentiality.
Practice Policies
Appointments and Cancellations
Cancel or reschedule at least 48 hours in advance. Full fee applies otherwise. Sessions are 50 minutes unless arranged otherwise. Returned checks incur a $20 fee.
Telephone Accessibility
Calls returned within 48 hours. Emergencies call 911. Phone sessions may be available within Nevada.
Social Media and Telecommunications
To protect confidentiality, I do not accept friend or contact requests on social media.
Electronic Communication
Electronic communication is not secure. Use only for scheduling. Telehealth follows Nevada law (NRS 629.515).
Minors
In Nevada, minors 12+ may consent to mental health treatment in certain cases. Parents are informed only when legally required or therapeutically necessary.
Termination
If no appointments are scheduled for three consecutive weeks without arrangement, treatment may be considered terminated. Referrals available on request.
Cancellation/No-Show Policy
Two late cancellations result in a warning. Three may result in termination. Fees still apply.
Billing & Credit Card Policy
By signing, you authorize charges via Stripe through SimplePractice. Disputes must align with these terms. Charges appear as ‘Ariana Throne LLC’.
Client Grievance Rights
If you have concerns, you are encouraged to first discuss them with your therapist. If unresolved, you may file a complaint with the Nevada Board of Examiners for Marriage and Family Therapists and Clinical Professional Counselors:

You may file a complaint without fear of retaliation. Clients have the right to services free from discrimination, abuse, neglect, or coercion.
Cancellation Policy
We understand the value of your time and the importance of maintaining consistent appointments for your well-being. To ensure the smooth functioning of our services and to respect the time of both our clinicians and clients, we have implemented the following no-show policy:
- Appointment Confirmation: Clients are responsible for confirming their appointments in advance. This can be done via phone, email, or any other communication method specified by the clinic.
- Late Arrival: Clients are expected to arrive on time for their scheduled appointments. The clinician will wait up to 15 minutes from the scheduled appointment time. If the client arrives within this grace period, the session will proceed as planned, and the full session fee will be charged.
- No-Show Fee: If a client fails to arrive for their scheduled appointment or cancels with less than 48 hours’ notice, they will be charged the full session fee.
- Cancellation Policy: Clients must provide at least 48 hours’ notice if they need to cancel or reschedule their appointment. Failure to do so will result in the application of the no-show fee.
- Responsibility for Communication: If the clinician is unable to attend a scheduled appointment, the client will be informed as soon as possible, and every effort will be made to reschedule at a convenient time. If the client arrives for a scheduled session and the clinician is not present, the client should contact the practice or clinician to inquire about the situation and reschedule if necessary.
- Courtesy Policy: We ask for the same courtesy from clients. If a client arrives for a scheduled appointment and finds the clinician is not present, they should wait up to 15 minutes, then contact the practice or clinician to inquire about the situation and reschedule if necessary.
- Repeated No-Shows: Repeated no-shows or late cancellations may result in the client being unable to schedule future appointments with the clinic. Two no-shows will result in a warning, and three may result in discharge from services.
We understand that emergencies and unforeseen circumstances may arise, and exceptions to this policy may be made at the discretion of clinic management. However, we encourage open communication and mutual respect for appointments to ensure the best possible care for all clients.
By scheduling an appointment with Ariana Throne, you acknowledge that you have read, understood, and agree to abide by the terms of this no-show policy. If you have any questions or concerns, please do not hesitate to contact us.
Intern Disclosure
You will be receiving treatment from a Registered Intern. Registered Interns are pre-licensed professionals who have graduated with at least a master’s degree in counseling and are working toward full licensure under the supervision of a Qualified Supervisor in the state in which they practice. Your counselor’s Qualified Supervisor’s name can be provided upon request.
By participating in treatment with this clinician, you consent to allow the Registered Intern to share verbal, written, and electronically recorded information with internship supervisors and other clinicians practicing at this site. All clinicians are held to the same ethical standards and will protect your name and other identifying information in accordance with HIPAA guidelines. All information you share is confidential, except in cases required by law. These exceptions are outlined in the Health Insurance Portability and Accountability Act (HIPAA) Client Rights & Therapist Duties form.
All Registered Interns follow the Nevada State Code of Ethics for Licensed Mental Health Counselors, Clinical Social Workers, and Marriage and Family Therapists.
